When completing a form, use black ink. Ink of any other color does not scan well into the electronic filing system.

Failure to timely file a required form may result in the inability of the Judge assigned to your matter to proceed to a final hearing until the required form(s) is/are completed and filed online, or filed with the Grant County Clerk of Court located on the second floor of the Grant County Courthouse.

Forms are available at wicourts.gov. Once completed, all forms shall be filed with the Grant County Clerk of Court located on the second floor of the Grant County Courthouse.
Some forms, such as the Financial Disclosure Statement [FA-4139V], may be mandatory and should be filed within the time required by statute [§767.127].
Some forms, such as the Parenting Plan [form FA-4147V], may be mandatory and should be filed within the time required by statute [§767.451(6m), and §767.41 (1m)] and with the DFCS at least 10 days before the first mediation session.
If parties resolve all divorce issues, a Marital Settlement Agreement (MSA) [form FA-4150V (with children) and FA-4151V (without children)] should be completed, signed, and then filed with the Grant County Clerk of Court. If a parent has opted in as an electronic party, the MSA and all other forms may be filed electronically.
